Visits can be used for maintenance for your body, improving performance, recovering from injury, or any other optimization of your health.

You can use your health savings account or flex spending accounts to purchase this membership.
If you have a physical therapy benefit with your health insurance, you can submit claims to your insurance carrier at the time of visits used during the year. Ability Physical Therapy will provide you with the appropriate paperwork to submit and you will be reimbursed based on your benefit.
You will be able to use the 12 visits in any combination throughout the year (i.e. once monthly, 6 visits in one month, or any combination to equal 12 visits).
Visits can be utilized to treat multiple body areas and/or for multiple episodes of care over the course of the year.
Membership can be shared with immediate family members and will not be transferred to other patients outside of your household.
Membership packages cost $1890.00. Packages can be paid in full or paid monthly at $157.50 (autopay).
